The Way Back Program offers thoughtful, hands-on support to help you (or someone you love) return home, get settled, and feel like yourself again.
Support That Starts Right Away and Stays With You
A hospital stay doesn’t pause our care. In fact, it deepens our connection. From the moment one of our residents is admitted, we stay involved, visiting in person within 24 hours and continuing to check in throughout their hospital stay. Whether it’s bringing along a favorite sweater or simply offering a familiar face, we’re there to provide both comfort and continuity.
Working Behind the Scenes to Smooth the Way
While making those visits, our leadership and wellness teams also work closely with hospital staff, discharge planners, and doctors. We coordinate care, attend planning meetings, and ensure everything is ready for your return so you can come home with confidence.
A Real Alternative to Skilled Nursing Facilities
Not everyone wants or needs to move to another care setting after a hospital stay. For many, staying in the place they know best is what helps healing most. That’s why we assess each resident’s changing needs and find ways to support them right where they are — in their own apartment, surrounded by the community they love.
