• The Personal Assistant provides and documents resident care. All employees are expected to model professionalism, compassion and will promote teamwork. The personal assistant is the first responder to resident calls and promptly reports changes of condition.


  • The Cook is responsible for preparing and serving food in a manner consistent with Blake Management Group’s standards of excellence. The cook is generally charged with preparing and plating meals on a day to day basis, but will also be required to assist with any and all dining events such as family gatherings and special occasions/holidays. Success of this position will be measured by resident satisfaction, organization, consistency, cleanliness, quality of work and timeliness in preparation.


  • The Concierge Reception Representative monitors the front entrance; greets and assists residents and guests; answers phones; assist with tours; and completes general administrative tasks which support the front office, marketing and all departments.


  • The Licensed Practical Nurse and/or the Registered Nurse is responsible for providing resident care and to perform those tasks that require a valid nursing license. General responsibilities include obtaining vital signs; administering medications; administering treatments; coordinating and documenting resident care; communicating with physicians and other health care providers; interacting with family members; and supervising the direct care provided by unlicensed personal care assistants. Nurses will perform only those tasks within the scope of their applicable board of nursing. All wellness employees report directly to the Director of Wellness. Licensed nurses may be required to be on-call and/or to participate in the manager-on-duty program.


  • The Memory Care Coordinator is responsible for the day to day operations and the provision of quality care to residents living in The Blake Memory Care Community. The Memory Care Coordinator hires, trains, supervises and schedules staff members. The Memory Care Coordinator alerts the Director of Wellness when a resident has a change of condition. General responsibilities include: coordinating services from dietary, maintenance and housekeeping departments; maintaining a clean and attractive environment; ensuring state and federal regulations are maintained; ensuring staffing levels remain above state regulations; utilizing the VIGIL dementia system to maintain a secure and safe environment; ensuring residents enjoy daily activities that are structured and well organized.


  • The Executive Director is responsible for the leadership, management and organization of the community. This responsibility includes the management of staff, budgets, services, marketing efforts and regulatory compliance. The Executive Director ensures that policies and procedures established by Blake Management are implemented and maintained consistently. The Executive Director is responsible for all day to day operations. All department directors report directly to the Executive Director.
